A special examination will be conducted on May 15, 2009 for PSU Lingayen Campus applicants.

ATTENTION: IN – COMING FRESHMEN / TRANSFEREES
After passing the CAT and INTERVIEW, the following are the STEPS to follow:
1. Present all the Entrance Requirements to the Program Adviser / Interviewer and secure your registration form.
2. Fill – up the form properly and copy your subjects.
3. Go back to the Program Adviser / Interviewer for Course Checking.
4. To the Internet Room (2nd Flr. CAS Building) for your Student Permanent Number (SPN) and Assessment of fees.
5. Pay your Tuition Fee and other miscellaneous fees at the Cashier’s Office.
6. Apply for your library card (bring 2 pcs. 1 x 1 ID picture) at the De Venecia Technology Information Center (DVTIC)
7. Proceed to ROTC Headquarters – for NSTP – ROTC and Prof. Robert Sison for NSTP – CWTS.
8. Proceed to the office of the Director for Student Services and Alumni Affairs (Main Bldg. – 2nd Floor) for you ID PICTURE – TAKING (Present your Official Receipt).
9. To the Registrar’s Office – for your submission of ALL ENTRANCE REQUIREMENTS:
a. Form 138 – High School Card – for Freshmen
Transfer Credentials / Cert. of Grades – for Transferees
b. Medical Certificate (Original)
c. NSO Birth Certificate (Xerox)
d. Certificate of Good Moral Character (Original)
e. 2 pcs. (2×2) ID pictures with name tag.
f. Long brown envelope with label (available at the PSU Cooperative Canteen)
